Overview
Case Managers and Care Coordinators are critical members of the Healthy Start (HS) team, working directly with individuals and families. They develop a strength-based, collaborative relationship with HS participants and their families to help meet their health and social needs. Case Managers and Care Coordinators may see participants virtually or in-person, and in their homes, HS project office, or other settings.
Case Management & Care Coordination Services
Case management/care coordination includes the following services:
- Screening and intake
- Comprehensive assessment of needs
- Creation of service care plan
- Identification of appropriate services
- Linkages to additional services
- Monitoring of progress
- Reassessment as needed
- Creation of discharge plan and case closure
